All forms for the Rural Health Care Program are filed through the online application management system My Portal. Access My Portal via the E-File system by clicking the "My Portal" button below.
A My Portal account is created by submitting an FCC Form 460 (HCF Program) or Form 465 (Telecom Program). Upon approval of the form, you will receive an automatic email with instructions for creating your My Portal password.
Reset your password by clicking “Reset Password” on the E-File login page. Your user ID is your email address.
Primary Account Holder: Each HCP has one primary account holder. The primary account holder has the ability to sign, certify, and submit forms for an HCP. The primary account holder must be a director, officer, or authorized employee of the HCP.
Secondary Account Holder: A secondary account holder also has the ability to sign, certify, and submit forms for an HCP. You can designate multiple secondary holders on your FCC Form 460 and/or 465. For the HCF program, the secondary account holder must be a director, officer, or authorized employee of the HCP.USAC recommends assigning at least one secondary account holder so that the HCP is still able to submit forms and meet deadlines should the primary account holder be unavailable.
Tertiary Account Holder (HCF Program Only): Tertiary account holders are third parties/consultants authorized to submit forms on behalf of an HCP. The third party must submit a Third Party Authorization before they can access My Portal.
